False PHD is a passive item introduced in The Binding of Isaac: Repentance expansion. Found primarily in Devil Rooms and Curse Rooms, it acts as the twisted counterpart to the original standard PHD item. Core Effects
Pill Identification: It automatically identifies the effects of all pills upon pickup, removing any guessing games.
Immediate Health: Grants one Black Heart immediately when you pick it up.
Negative Pill Force: It forces all positive stat pills to convert into their negative equivalents. The Twist (Stat Downs into Damage Ups)
While it converts your pill pool into negative pills, it gives those negative pills massive secondary buffs:
Stat-Down Pills: Any pill that lowers a stat (like Tears Down, Speed Down, or Luck Down) will also grant you a permanent +0.6 Damage Up.
Non-Stat Negative/Neutral Pills: Pills with negative or abstract effects that don’t reduce core stats (such as Amnesia, Addicted, R U A Wizard?, or Paralysis) will spawn a Black Heart on the floor instead.
True Neutral Exceptions: Strictly neutral cosmetic pills like “I Found Pills” or “Relax” will not grant bonuses. Top Synergies
Rock Bottom: This prevents your stats from ever dropping while still allowing the False PHD to grant you the flat damage upgrades.
Standard PHD / Lucky Foot / Virgo: If you hold these alongside False PHD, the “all pills are bad” rule is nullified. Good and bad pills will both spawn naturally, but using any bad pill still triggers the False PHD damage/health bonus.
High Damage Multiplier Characters: Characters like Judas or Tainted Azazel scale incredibly well with raw damage increases, making the stat-for-damage trade completely worth it.
